Magická kostka, známá z obou předchozích filmů, se vrací ukryta v bizardním sousoší, jež koupí majitel baru Kotelna, cynický J. P. Monroe. O jeho podnik se začne zajímat televizní reportérka Joey, která se stane svědkem podivné smrti jednoho mladíka. V galerii pak objeví sochu pocházející ze zrušeného ústavu doktora Channarda, jejíž součástí je Pinheadova hlava. Socha ožívá. Krutý boj s Pinheadem znovu začíná a bude to tentokrát Joey, kdo se s ním utká tváří v tvář.
